Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs and estimated insurance benefits before treatment is administered. Charges are payable at the time of treatment.
We accept cash, checks, debit cards, Visa, MasterCard, Discover, American Express and also offer out-of-office financing through CareCredit®.
If special arrangements are needed, please request more information from our financial coordinator prior to receiving service.
As a courtesy to our patients, we submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
Important information to United Concordia PPO members:
As of January 1, 2015, we will no longer be contracted as participating providers with United Concordia Co. Inc. PPO network.
After the termination effective date of January 1, 2015, as a United Concordia member, we will continue to provide quality care. As an out-of-network provider, receiving dental care at Judd Dentistry may result in additional patient financial responsibilities based upon the terms of the benefit plan.
As a courtesy, we will continue to submit the dental claims, however, payment for services will be collected in full, at the time of the service and the insurance company will reimburse you directly.
